This form should be completed by a coach or manager with the authority to commit the team to participating in the tournament. Once completed, the form must be submitted to KMHATournament@gmail.com and payment by e-transfer to generalmanagerkmha@outlook.com in order to be able to confirm your spot in the tournament.

The cost to participate is $1275 CAD.

The tournament is open to House League teams at the A(Tier 1), B(Tier 2) and C(Tier 3) levels (there will be separate divisions) and there will be a 3 game guarantee in the round robin followed by a playoff.
